Dementia  Specific  Long  Term  Care  Facilities

For  some  individuals  with  Alzheimer’s  disease  or  a  related  dementia,  placement  in  a  
long-term  care  (LTC)  facility  may  be  considered.    This  decision  can  be  difficult  and  
emotional  for  both  the  individual  with  Alzheimer’s  disease  and  the  family/caregivers.    
However,  a  long  term  care  facility  can  offer  the  appropriate  medical  care,  intensive  

supervision,  dementia-specific  activities,  assistance  with  activities  of  daily  living  in  a
home  like  environment,  and  allow  family  and  caregivers  to  focus  on  their  personal  
relationship  rather  than  their  direct  caregiver  role.      

Ideally,  planning  for  a  move  to  a  LTC  facility  should  occur  several  months  in  
advance.    This  allows  time  to  research  facilities  in  your  area,  better  understand  the  
services  offered,  address  any  financial  issues  related  to  paying  for  a  LTC  facility,  
and  what  options  best  meet  both  family/caregiver  needs  and  the  individual  needs  of  
the  person  with  Alzheimer’s  disease.  


There  are  two  main  types  of  LTC  facilities,  assisted  living  facilities  and  skilled  
nursing  facilities.  There  are  also  adult  family  care  homes,  which  are  smaller  in  
nature  and  do  not  provide  as  many  services.  


​​Assisted  Living  Facilities:  


An  Assisted  Living  Facility  is  often  referred  to  as  an  ALF.    An  ALF  provides  housing,  meals,  

and  one  or  more  personal  services  for  a  resident.    
To  become  a  resident  of  an  ALF,  a  person  must  meet  state  mandated  functional  
criteria,  such  as  being  able  to  perform  activities  of  daily  living  independently  or  with  
supervision/assistance  (i.e.  bathing,  toileting,  or  walking).    An  ALF  resident  cannot  
be  bedridden  or  require  24-hour  nursing  supervision.    ALF’s  often  have  many  levels  
of  care  from  minimal  to  comprehensive  to  meet  the  needs  of  each  individual  
resident.    Some  facilities  also  offer  dementia-specific  units  that  provide  services  
aimed  at  meeting  the  unique  needs  of  individuals  with  Alzheimer’s  disease.    

Dementia-specific  care  usually  includes  higher  staff  to  resident  ratios,  staff  
specifically  trained  in  working  with  Alzheimer’s  disease,  dementia  appropriate  
activities,  safe  areas  for  residents  to  wander  independently,  and  security  measures
to  prevent  unsupervised  wandering  or  elopement. 
ALF’s  are  licensed  and  inspected  by  the  State  of  Florida,  and  anyone  touring  the  
facility  should  request  disclosure  of  the  most  recent  inspection  documents  to  inquire  
about  any  deficiencies.    Although  an  ALF  does  not  provide  24-hour  nursing  
supervision  or  total  care,  some  ALF’s  do  have  special  Extended  Congregate  Care  
licenses  (ECC),  Limited  Nursing  Services  licenses,  or  Limited  Mental  Health  
licenses  to  allow  them  to  provide  expanded  health  and  mental  health  services  not  
normally  provided  in  an  ALF.  

Residency  in  an  ALF  can  be  paid  for  privately  or  through  a  long  term  care  insurance  
policy.    The  State  of  Florida  also  offers  a  Medicaid  Assisted  Living  for  the  Elderly  
Waiver  (resident  must  qualify  for  the  waiver  and  the  facility  must  accept  the  waiver)  
which  helps  pay  a  portion  of  the  monthly  ALF  cost,  as  well  as  offering  some  monthly  
assistance  paying  for  continence  supplies.  The  Veteran’s  Administration  also  offers  
assistance  to  veterans  and  their  dependents  in  paying  for  assisted  living  through  the  
Aid  &  Attendance  Program.    You  may  apply  for  Aid  &  Attendance  by  calling  1-800-  
827-1000,  by  talking  to  your  VA  social  worker  if  already  receiving  VA  medical  
benefits,  or  by  going  to  your  local  Veteran’s  Service  Center  (see  offices  below).  

Skilled  Nursing  Facility:    A  Skilled  Nursing  Facility  is  often  referred  to  as  a  SNF  
(pronounced  “sniff”)  or  a  nursing  home.    A  SNF  provides  nursing  care,  personal  
care,  custodial  care  and  rehabilitation  to  individuals  who  have  an  illness,  are  
recuperating  from  surgery  or  an  injury,  or  are  medically  fragile  and  require  24  hour  
nursing  supervision.    Some  SNF’s  have  specialized  dementia  units  that  offer  a  
higher  staff  to  patient  ratio,  staff  trained  to  work  with  individuals  who  have  
Alzheimer’s  disease,  safe  areas  to  wander,  dementia  appropriate  activities  and  
security  measures  to  ensure  a  patient  does  not  wander  or  elope  from  the  facility.    All  
SNF’s  are  licensed  and  inspected  by  the  State  of  Florida.    You  can  see  a  facility’s  
most  recent  inspection,  or  view  nursing  homes  on  a  “Nursing  Home  Watch  List”  by  
going  to  www.floridahealthfinder.gov  or  by  calling  the  Florida  Agency  for  Healthcare  
Administration  at  1-888-419-3456.    SNF’s  that  provide  Medicare  approved  services  
have  their  Medicare  inspection/survey  results  posted  on  www.medicare.org

SNF  care  can  be  paid  for  privately,  through  long  term  care  insurance,  Medicaid,  or  
by  Medicare  for  short  term  inpatient  rehabilitation  or  recuperation  related  to  a  recent  
hospitalization  of  more  than  72  hours.    The  Veteran’s  Administration  also  has  a  
dementia  specific  unit  in  Broward  County  that  is  available  to  Veterans.    To  apply  for  
Medicaid,  please  call  your  local  CARES  office.      

Adult  Family  Care  Home:  An  adult  family  care  home  (AFCH)  provides  a  full-time,  
family-type  living  arrangement  in  a  private  home  for  up  to  five  frail  elders  or  people  
with  disabilities,  who  are  not  related  to  the  owner.  To  qualify  for  an  AFCH  license,  
the  AFCH  provider  must  reside  in  the  same  house  as  the  residents.  

A  person  can  become  a  resident  of  an  AFCH  only  if  certain  functional  criteria  are  
met,  such  as  being  able  to  perform  the  activities  of  daily  living  (with  supervision  or  
assistance)  like  eating,  walking  and  toileting.  A  resident  cannot  be  bedridden,  
cannot  require  24-hour  nursing  supervision,  cannot  have  stage  3  or  4  pressure  
sores,  and  must  meet  other  criteria  as  described  in  Chapter  429,  Part  II,  Florida  
Statutes  and  Chapter  58A-44,  Florida  Administrative  Code.  

The  AFCH  provides  housing,  meals,  personal  services,  health  monitoring,  and  can  
provide  or  assist  in  arranging  for  nursing  services  for  the  resident  when  needed,  as  
long  as  the  resident's  health  does  not  require  more  care  than  can  be  provided.  
Additional  services  can  include  arrangement  of  transportation  to  medical  
appointments,  maintenance  of  clothing,  securing  social  activities  and  leisure  
services,  and  arrangement  for  participation  in  religious  activities  if  requested  by  the  
resident.  

Aging  and  Disability  Resource  Center:  

The  State  Medicaid  Managed  Care  Program  provides  community-based  services, 
Assisted  Living  and  Nursing  Home  assistance  to  frail  functionally  impaired  seniors 
and  disabled  adults,  age  18  and  above  at  risk  of  nursing  home  placement.  The 
program  assists  clients  to  remain  in  their  own  homes  or  Assisted  Living  facilities.    

The  program  also  manages  clients  under  Long  Term  Care  Medicaid  in  a  Nursing 
Home.    Clients  must  meet  financial  eligibility  as  determined  by  the  Department  of 
Children  and  Families  and  functional  level  of  care  as  determined  by  CARES.   
Clients  choose  a  Long  Term  Care  Managed  Care  Plan  through  an  enrollment  
broker.      The  plan  assigns  a  case  manager  to  each  client  who  will  help  develop  a 
care  plan.  

The  Medicaid  Long  Term  Care  program  is  administered  and  managed  by  the 
Department  of  Elder  Affairs,  Area  Agencies  on  Aging,  Department  of  Children  & 
Families,  the  Agency  for  Health  Care  Administration  and  four  Managed  Care  Plans 
which  for  PSA  9  include:    

    American  Eldercare  
    Coventry  Health  Care 
    Sunshine  State  Health  
    United  Healthcare
